NWAL-WV Shetland Sheepdog Dog Rescue, Inc. does not charge for dogs placed in new homes.  Therefore, to continue our work we ask for a donation of $265 for puppies, $225.00 for shelties one (1) year or older, and $165 for sheltie mixes, so that we can help other Shetland Sheepdogs in need. In the case that an animal is returned, we have a NO REFUND policy unless the animal is proven to have had an illness which happened during the time the dog is the property of the rescue group.

I understand that there will always be an adjustment period for any dog coming into my home. The average is three (3) weeks to three (3) months and I am willing to work to make this pet a member of the family. I also understand that many of these dogs have survived tremendous odds and need lots of TLC.

I understand that, the NWAL-WV Shetland Sheepdog Dog Rescue, Inc. reserves the right to check on the welfare of this dog on my premises, and in the event I do not comply with the adoption contract provisions and the NWAL-WV Shetland Sheepdog Dog Rescue, Inc. reclaims the pet, I waive claims for my right to the dog. I also agree that if the information on this questionnaire is not accurate to the best of my knowledge, I forfeit all rights and ownershipof the dog adopted from the NWAL-WV Shetland Sheepdog Dog Rescue, Inc..I also agree that if at any time in the future I can no longer keep this dog, I will not give it away or sell it. I will contact the NWAL-WV Shetland Sheepdog Dog Rescue, Inc. for arrangements to be made for the dog's return to it.
I have read completely the above questionnaire, and understand it fully.

In consideration of donation of($______Dollars), the receipt of which is hereby acknowledged Adopter(s) do hereby release, discharge and hold harmless NWAL-WV Shetland Sheepdog Dog Rescue, Inc., hereafter “NWALWVSSR, Inc.”, and any other person, firm or corporation charged or chargeable with liability, their heirs, administrators, executors, successors and assigns, from any and all claims, damages, costs, expenses, loss of services, actions and causes of action belonging to the said Adopter(s) arising out of any act or occurrence from the present time, and particularly on account of the adoption of ____________,

Adopter(s) hereby declare that no representations about the nature of this dog, nor any representations regarding the nature and extent of legal liability of financial responsibility have induced the Adopter(s) to make this Release and Indemnity Agreement. The UNDERSIGNED hereby further agrees to abide by the “Terms of Release” listed below:

Identification tags in according to laws of state
Notification if dog is lost or stolen
Will keep dog for natural lifetime or return to above rescue group
Give annual physical, vaccines and heartworm preventative
Provide, food, water and shelter (no chains or chain runs)
If adopted a puppy, will sterilize by age 4-5 months
Dog will not roam free. Will be kept safe at all times
Will be given basic obedience training. No attack or guard dog training
Will not transport dog in open bed of pickup. Will crate or secure with harness in vehicle

HEALTH PROGRAM - The dog will be given an ANNUAL heartworm check and maintained on a quality preventative, &/or a program established by the Adopter(s) veterinarian for the rest of the dog's natural life. The Adopter(s) will provide a high quality dog food, water, shelter, protection and any medical care needed. The Adopter(s) understand that the dog will be up to date on all vaccinations & on heartworm preventative, however, accepts the risk of the dog having any congenital problems or previously contracting rabies or any other infectious diseases or heartworms and hereby releases NWALHDR of any liability or results thereof.

Heartworm Warning
It has been discovered that a dog can test negative for Heartworms, but 6 months later test positive.  This is because the heartworms had not developed enough to show up when the first test was taken. It is not NWAL-WV Shetland Sheepdog Dog Rescue, Inc.’s fault if the dog test positive for Heartworm at a later date & NWAL-WV Shetland Sheepdog Dog Rescue, Inc., cannot be held financially liable. We have taken every precaution to prevent this from happening, but it could still happen. You should also have a fecal exam done on a regular basis & shots updated every three years, until age 7 and then discontinue shots except Rabies. And it is very important to keep your dog on Heartworm Preventative program, so he/she does not get Heartworms.

All of the above information I have given is true and complete. Should an unneutered or unspayed dog
be placed with me, I agree to have it neutered or spayed within one month of adoption or by a date agreed upon by me and a Rescue Representative of NWAL-WV Shetland Sheepdog Dog Rescue, Inc..This dog will reside in my home as a pet. I will provide it with adequate food, water, shelter, training, affection, and medical care. I understand that NWAL-WV Shetland Sheepdog Dog Rescue, Inc. is not responsible for the accuracy of information received about the habits, temperament, or physical condition of dogs available for adoption. I understand that it is my responsibility to see and evaluate the dog for myself before agreeing to adopt it. I am in full agreement with these terms of adoption. NWAL-WV Shetland Sheepdog Dog Rescue, Inc. is in no way liable or responsible for any damage, accident or injury resulting from the placement of a dog into my household.
